RESOLUTION AUTHORIZING THE EXECUTION OF A SECOND AMENDMENT TO THE AGREEMENT WITH THE STATE OF CALIFORIA, DEPARTMENT OF REHABILITATION FOR EMPLOYMENT AND TRAINING SERVICES TO THE DISABLED POPULATION IN SAN MATEO COUNTY

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the Human Services Agency and the State of California, Department of Rehabilitation have joined in partnership and developed an Agreement for the rehabilitation of consumers with disabilities in San Mateo County; and

WHEREAS, pursuant to the Agreement, the County will provide vocational assessment, employment preparation, job development and placement services for populations with significant physical and psychiatric disabilities not receiving services from County Mental Health; and

WHEREAS, the County and the State Department of Rehabilitation wish to amend the Agreement to increase funding from the Department of Rehabilitation by $38,092 changing the Department of Rehabilitation’s total Agreement obligation from $494,092 to $532,184 with the term of the Agreement remaining July 1, 2004 through June 30, 2007, and the County’s match will change from $80,604 to $88,718 per fiscal year for the purpose of funding rising program and staff costs in FY 2005-06 so additional Mental Health consumers can receive services; and

WHEREAS, this Board of Supervisors acknowledges the benefits and responsibilities to be shared by both parties to said Agreement, and this Board has been presented with a form of this Second Amendment to the Agreement and has examined, and approved it as to both form and content, and, desires to enter into same.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said Second Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
