RESOLUTION A) WAIVING THE REQUEST FOR PROPOSAL (RFP) PROCESS AND B) AUTHORIZING THE EXECUTION OF AN AGREEMENT WITH RAVENSWOOD FAMILY HEALTH CENTER
TO PROVIDE PRIMARY CARE SERVICES TO
SAN MATEO COUNTY’S HOMELESS POPULATION

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, San Mateo Medical Center was awarded the Health Care for the Homeless (HCH) grant from the federal Bureau of Primary Health Care for the Health and Resource Services Administration to provide primary care, dental care, outreach, health education, case management, and supportive services to the San Mateo County’s homeless population; and

A Request for Proposal process followed and based on the priorities of the strategic plan, Ravenswood was awarded a contract to provide primary care services to 600 homeless individual for the term November 1, 2004 to October 31, 2005, for a maximum amount of $98,997; and

WHEREAS, Ravenswood Family Health Center exceeded its objectives and goals for FY2004-05 and has been approved to provide primary care to 700 homeless individuals for FY2005-06; and

WHEREAS, both parties now wish to enter in an Agreement, to provide primary care to San Mateo County’s homeless population, for the term November 1, 2005 to October 31, 2006, for a total obligation of $108,000; and

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to waive the RFP process and execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
