RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT WITH

FRED LUI, MD FOR NEPHROLOGY SERVICES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on August 4, 2005 the County of San Mateo, by Agreement No. C246, awarded a contract to Fred Lui, MD in the amount of $60,000 for the term April 1, 2005 through September 30, 2005 for the purpose of providing nephrology services at the San Mateo Medical Center; and

WHEREAS The contract with Fred Lui, MD expired on September 30, 2005 and the San Mateo Medical Center has a continued need for nephrology services; and

WHEREAS, San Mateo Medical Center now wishes to enter into an Agreement with Fred Lui, MD to provide nephrology services for the period October 1, 2005 through September 30, 2008 for a total obligation of $360,000; and

WHEREAS there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made, whereby Fred Lui, MD shall continue to provide nephrology services; and

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
