RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH BAY AREA UROLOGY GROUP
FOR UROLOGY SERVICES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that
|
|
|
WHEREAS, on June 29th, 2004 the Board of Supervisors, by Resolution 66774, authorized an Agreement with Bay Area Urology Group in the amount of $474,398.68 for the term June 1, 2004 through December 31, 2005 for professional urology services to the San Mateo County Medical Center; and
|
|
|
WHEREAS, the parties now wish to amend the previous Agreement by extending the length of the contract by two months from June 1, 2004 to December 31, 2005 to June 1, 2004 through February 28, 2006; and
|
|
|
WHEREAS there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment, reference to which is hereby made, whereby Bay Area Urology Group shall continue to provide professional urology services through February 28, 2006; and
|
|
|
WHEREAS, this Board has been presented with a form of the Amendment and has examined and approved it as to both form and content and desires to enter into the Amendment:

|
NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
