RESOLUTION AUTHORIZING THE EXECUTION OF A SECOND AMENDMENT TO THE AGREEMENT WITH EDGEWOOD CENTER FOR CHILDREN AND FAMILIES SERVICES DECREASING THE MAXIMUM OBLIGATION BY $211,590 TO A NEW MAXIMUM OF $1,217,041, FOR THE TERM JULY 1, 2004 THROUGH JUNE 30, 2006

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a second amendment to the Agreement, reference to which is hereby made for further particulars, whereby Edgewood Center for Children and Families shall provide school based mental health services for mentally ill children and youth, outpatient mental health services to youth and their families through the Mental Health Plan, and Child Abuse Treatment Program services; and

WHEREAS, both parties now wish to amend the Agreement to define Child Abuse Treatment Program administrative services, to revise the volume of Child Abuse Treatment Program services, and to decrease the maximum obligation by $211,590 for a new maximum of $1,217,041 for the term July 1, 2004 through June 30, 2006; and

WHEREAS, this Board has been presented with a form of the second amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the second amendment to the Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is authorized to execute said second amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
