RESOLUTION AUTHORIZING THE PURCHASING AGENT TO EXECUTE A CHANGE NOTICE TO VENDOR AGREEMENT #B404009 DATED NOVEMBER 1, 2003 WITH GEN-PROBE, INC.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a request to allow the Purchasing Agent to execute a change notice to the vendor agreement with Gen-Probe, Inc., reference to which is hereby made for further particulars; and

WHEREAS, both parties now wish to modify the vendor agreement to continue purchasing supplies through October 31, 2008; the amount of these supplies will not exceed $400,000, total amount for the contract term will not exceed $1,000,000; and

WHEREAS, this Board has been presented with this request to allow the Purchasing Agent to extend said vendor agreement and it has been approved as to both form and content and this Board desires to enter into said Extension.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby, authorized and directed to allow the Purchasing Agent to extend said vendor agreement and purchase orders on behalf of the County of San Mateo and the Clerk of this Board shall attest the President’s signature thereto.
