RESOLUTION AUTHORIZING THE EXECUTION OF A SECOND AMENDMENT TO THE AGREEMENT WITH THE DEPARTMENT OF REHABILITATION

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, The Board of Supervisors authorized the execution of an Agreement issued by the Department of Rehabilitation by Resolution Number 067074 for the purpose of funding situational assessment, work adjustment, employment preparation, job development and placement services for County Mental Health consumers in the amount of $767,190, which provided funding to the Human Services Agency for FY 2004-05 services only, with funding for future years in the form of Amendments, and with a term of July 1, 2004 through June 30, 2007; and

 

WHEREAS, a First Amendment to the Agreement was authorized by Resolution Number 067466 for the purpose of adding funds for FY 2005-06 in the amount of $767,190 changing the Agreement obligation from $767,190 to $1,534,380 and adding additional monitoring and reporting provisions, to which the County agreed; and

WHEREAS, the County and the Department of Rehabilitation wish to enter into a Second Amendment for the purpose of the Department of Rehabilitation’s appropriation of additional funds to the Human Services Agency in the amount of $81,310 for a total obligation of $1,615,690 for the purpose of providing services to additional participants in FY 2005-06.

 

WHEREAS, this Board has been presented with a form of this Second Amendment to the Agreement and has examined and approved said Second Amendment as to both form and content and desires to enter into same.

 

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Second Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
