RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT WITH
PACIFIC FOOTCARE PODIATRY GROUP, INC. FOR
PODIATRY SERVICES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on April 16th, 2002 the Board of Supervisors, by Resolution 66195, authorized an Agreement with Richard Woo, DPM, to provide podiatry services to the San Mateo Medical Center for the term April 1, 2002 through March 31, 2005; and

WHEREAS, the County Manager, by Contract number 66000-05-C-250, authorized an Agreement with Richard Woo, DPM , to provide podiatry services for the term April 1, 2005 through September 30, 2005 while new contract terms were negotiated; and

WHEREAS, the San Mateo Medical Center has a continuous need for podiatry services and the Pacific Footcare Podiatry Group, Inc. under Richard Woo, DPM would expand provider coverage of uninterrupted podiatry clinic and surgical services; and

WHEREAS, the parties now wish to enter into an Agreement for the term of October 1, 2005 to September 30, 2007 with a maximum reimbursement amount of $179,664.50; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made, whereby Pacific Footcare Podiatry Group, Inc. shall provide professional podiatry services; and

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
