RESOLUTION AUTHORIZING THE ACCEPTANCE OF A TWO YEAR GRANT FROM THE LUCILE PACKARD FOUNDATION FOR CHILDREN’S HEALTH FOR THE TERM OF NOVEMBER 2005 TO NOVEMBER 2007

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors a request to accept a two year grant from the Lucile Packard Foundation for Children’s Health Grant Number 939 in the aggregate amount of $80,000 to provide enhanced prevention and early intervention services for children aged 0-5 in Daly City and Redwood City; and

WHEREAS, the term of the grant is November 2005 to November 2007; and

WHEREAS, the Lucile Packard Foundation for Children’s Health’s grant offer letter is attached hereto and the Board has examined and approved the terms and conditions of the grant desires to accept the same;

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to accept the Grant from the Lucile Packard Foundation for Children’s Health for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
