RESOLUTION AUTHORIZING THE EXECUTION OF AN AMENDMENT TO THE AGREEMENT BETWEEN THE COUNTY OF SAN MATEO AND ADVANCED TOTAL IMAGING, INC.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on September 27, 2005, the Board of Supervisors executed an Agreement, in the amount of $400,000 for the term of October 1, 2005 to March 31, 2006, whereby Advanced Total Imaging, Inc. agreed to provide scanning services to the Human Services Agency for the conversion of MediCal case files, including materials and workmanship as follows: (1) Pick-up and preparation of case files to include barcode pages (2) Scanning services (3) DVD delivery, and (4) Shredding services.

WHEREAS, the parties now wish to amend the Agreement with Advanced Total Imaging, Inc. to extend the term of the Agreement to December 31, 2006; and

WHEREAS, this Board has been presented with a form of such Amendment, and said Board has examined and approved it as to both form and content and desires to enter into same.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo; and the Clerk of this Board shall attest the President's signature thereto.
