RESOLUTION AUTHORIZING 1) AMENDMENT ONE TO THE AGREEMENTS WITH ALZHEIMER’S ASSOCIATION OF NORTHERN CALIFORNIA; AVENIDAS; CATHOLIC CHARITIES, CYO; CENTER FOR INDEPENDENCE OF THE DISABLED; CITY OF DALY CITY; CITY OF EAST PALO ALTO; CITY OF MENLO PARK; CITY OF PACIFICA; CITY OF SAN BRUNO; CITY OF SAN MATEO; CITY OF SOUTH SAN FRANCISCO; COASTSIDE ADULT DAY HEALTH CENTER; FAMILY SERVICE AGENCY; MILLS-PENINSULA SENIOR FOCUS; PENINSULA VOLUNTEERS, INC.; SECOND HARVEST FOOD BANK; SELF-HELP FOR THE ELDERLY; SENIOR COASTSIDERS; AND SEQUOIA HOSPITAL FOR THE PROVISION OF SUPPORTIVE, NUTRITION AND COMMUNITY-BASED SERVICES, INCREASING THE AGGREGATE TOTAL AMOUNT BY $267,043 TO $2,994,942 , WITH NO CHANGE TO THE TERM OF JULY 1, 2005 THROUGH JUNE 30, 2006 AND 2) AMENDMENT TWO TO THE AGREEMENT WITH OMBUDSMAN SERVICES OF SAN MATEO COUNTY, INC., FOR OMBUDSMAN SERVICES INCREASING THE TOTAL AMOUNT BY $7,000 TO $300,581, WITH NO CHANGE TO THE TERM OF JULY 1, 2005 THROUGH JUNE 30, 2006

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that
|
WHEREAS, this Board has designated itself as the Area Agency on Aging (AAA) of San Mateo County to carry out programs pursuant to the Older Americans Act (OAA);
|
|
WHEREAS, it is necessary and desirable that the AAA administer the Title III/VII/V of the OAA and Community-Based Services Programs (CBSP);
|
|
WHEREAS, on July 26, 2005, by Resolution Number 67461, the Board of Supervisors authorized execution of 21 Agreements in an aggregate total of $2,569,922, for the term July 1, 2005 through June 30, 2006 to provide an array of services to seniors and people with disabilities in San Mateo County;
|
|
WHEREAS, on November 10, 2005, by Resolution Number 67683, the Board of Supervisors authorized execution of an Agreement with Self-Help for the Elderly in the amount of $376,558 for the term of July 1, 2005 through June 30, 2006;
|
|
WHEREAS, on December 12, 2005, by Resolution Number 67685, the Board of Supervisors authorized execution of Amendment One to the Agreement with Ombudsman Services of San Mateo County, Inc., increasing the amount by $75,000 to $293,581, with no change to the original term;
|
|
WHEREAS, all parties wish to further amend the Agreements by increasing the aggregate amount by $274,043, thus changing the County’s aggregate fiscal obligation for the 22 Agreements from $3,021,480 to $3,295,523, as set forth in Exhibit A to this resolution, with no change to the term of the Agreements, and

WHEREAS, the Board has been presented with 1) Amendment One to the Agreements with Alzheimer’s Association of Northern California; Avenidas; Catholic Charities, CYO; Center for Independence of the Disabled; City of Daly City; City of East Palo Alto; City of Menlo Park; City of Pacifica; City of San Bruno; City of San Mateo; City of South San Francisco; Coastside Adult Day Health Center; Family Service Agency; Mills-Peninsula Senior Focus; Peninsula Volunteers, Inc.; Second Harvest Food Bank; Self-Help for the Elderly; Senior Coastsiders; and Sequoia Hospital and 2) Amendment Two to the Agreement with Ombudsman Services of San Mateo County, Inc., and desires to enter into same:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said Amendments to the Agreements for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
