Acceptance of a Grant Award from California Department of Health Services

 

RECOMMENDATION:

A.

Adopt a Resolution:

   
 

1.

Accepting a grant from the California Department of Health Services in an amount not to exceed $175,000 to participate in a program entitled “Best Practices in Childhood Asthma” (BPCA), providing clinical case management to children with asthma who are patients at San Mateo Medical Center (SMMC) outpatient clinics in the south county region.

   
 

2.

Authorizing the execution of an Agreement with the California Department of Health Services to participate in the BPCA Program for the term March 1, 2006, to June 30, 2008, in an amount not to exceed $175,000.

   

B.

Approve an Appropriation Transfer Request (ATR) to accept unanticipated revenue (in an amount not to exceed $35,000) for FY2005-06 and appropriate new revenue to Fair Oaks Children’s Clinic in the amount of $35,000.

This grant contributes to this commitment and goal by giving SMMC the opportunity to provide clinical case management and intensive support to children with asthma in the community to help them avoid hospitalization and self-manage their disease.

A total of 250 patients will be enrolled during the term of the Program. It is anticipated that 80% of children participating in Program will show reduction in clinical symptoms, missed school days, exacerbation episodes and emergency room visits.

 

BACKGROUND:

The SMMC Pediatric outpatient clinics at Fair Oaks, Sequoia Teen Wellness Center and Willow have been providing enhanced case management services to children with asthma during the past year with funding from California HealthCare Foundation. The program has developed an electronic patient registry and patient self-management using mobile phones. Case management is the key to the program’s success, but funding is needed to sustain the case management position for the next 3 years.

 

DISCUSSION:

In November 2005, CA Department of Health Services announced the availability of funds to support case management as a way to provide Best Practices in Childhood Asthma care. On December 1, 2005, SMMC submitted a proposal in response to this announcement and was awarded a three year grant in an amount not to exceed $175,000. The County will receive an amount not to exceed $35,000 for FY2005-06, because the state delayed commencement of the project until March 1. The rest of the funds will be disbursed as $70,000 in FY2006-07 and $70,000 in FY2007-08, pending legislative renewal.

The primary purpose of this program is to provide intensive case management services to children diagnosed with asthma at three SMMC pediatric clinics, in association with other technological and medical interventions. Ultimately the intention is to spread this program model to other county clinics and for other chronic disease conditions and age groups. As a result of this program, it is anticipated that children with severe or persistent asthma will experience a reduction in symptoms and hospitalizations, a higher level of daily functioning and greater participation with their medical provider in the management of their own condition.

 

FISCAL IMPACT:

The grant from California Department of Health Services will not exceed $35,000 during this fiscal year or $175,000 over the entire term of the grant. The ATR will add $35,000 to the FY2005-06 Budget. The term of the Agreement is March 1, 2006 to June 30 2008. Administrative oversight, technical assistance, and project monitoring will be provided by existing SMMC Clinics staff. There is no net county cost associated with this project.
