Amendment to the Agreement with Duc Marcel Nguyen, MD for Orthopedic Services

 

RECOMMENDATION:

Adopt a resolution authorizing the President of the Board to execute an Amendment to the Agreement with Duc Marcel Nguyen, MD to provide orthopedic services increasing the maximum Agreement obligation by $153,093.16 from $810,937.34 to $964,030.50.

BACKGROUND:

Dr. Duc Marcel Nguyen has provided orthopedic services to the patients of San Mateo Medical Center for the last several years. In particular, these services include the treatment for injury and diseases of the musculoskeletal system, including arthroscopic knee surgery, hip replacement, rotator cuff repair (knees and shoulders), bone breaks and fractures, and arthritis. Patients treated at SMMC have a continuous need for such orthopedic care services and Dr. Nguyen offers the hospital the benefits of an experienced provider familiar with the complex nature of the County’s patient base.

 

DISCUSSION:

In 2004 bone injury, trauma, disease, and surgery services for San Mateo Medical Center were contracted among four orthopedic surgeons. One of the contractors terminated his contract with the County effective January 1, 2006. Dr. Nguyen increased his availability to cover the service gap such that his current productivity levels will double in volume.

The original Agreement budget was based on an annual productivity level of 4000 RBRVS work units with a cushion to cover anticipated increases in surgical cases. Actual and projected productivity levels for the contractual period are anticipated at an increase of 80% of the original amount to reflect Dr. Nguyen’s assumption of the terminated contractor’s workload. Funds budgeted for the contract terminated January 1, 2006 are being shifted to cover the increased financial obligation for this contract.

Orthopedic services are provided under the general direction of the Chief of Medicine. Monitoring and evaluation of the quality of services provided by contractor are accomplished through peer review by the Quality Assurance Committee and the Executive Committee of the medical staff.

 

FISCAL IMPACT:

The Agreement is being amended to increase the maximum contract amount by $153, 093.16. The maximum amount the County shall be obligated to pay Contractor is increased from $810,937.34 to $964,030.50. Funds were included in the San Mateo Medical Center’s FY2005-06 Adopted Budget and will be included in the future fiscal year budgets.
