RESOLUTION AUTHORIZING 1) AN AMENDMENT TO THE AGREEMENT WITH ESTELA AGUILAR DBA ESTELA’S PLACE INCREASING THE AGREEMENT MAXIMUM BY $22,326 TO A NEW MAXIMUM OF $108,336 WITH NO CHANGE TO THE TERM, AND 2) AUTHORIZING THE DIRECTOR OF HEALTH TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000 IN AGGREGATE AND TO MAKE MINOR CHANGES IN THE TYPE OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on November 15, 2004, the County Manager approved an Agreement, reference to which is hereby made by further particulars, whereby Estela Aguilar dba Estela’s Place provides board and care services for the term July 1, 2004 through June 30, 2007 for a maximum obligation of $86,010; and

WHEREAS, both parties now wish to enter into an amendment to the Agreement to increase the Agreement maximum obligation $22,326, for a total obligation of $108,336 with no change to the Agreement term; and

WHEREAS, this Board has been presented with the amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the amended Agreement;

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized to execute said amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED, the Director of Health is authorized to execute subsequent amendments and minor modifications to the amendment to the Agreement not to exceed $25,000 in aggregate and to make minor changes in the types of services and activities under the Agreement.
