BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING (1) A SECOND AMENDMENT TO THE AGREEMENT WITH NORTH PENINSULA NEIGHBORHOOD SERVICES CENTER, FOR CORE SAFETY NET SERVICES TO PARTIALLY RESTORE CORE SERVICES TO PRIOR FUNDING LEVEL; AND (2) THE DIRECTOR OF THE HUMAN SERVICES AGENCY TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on September 27, 2005, the Board of Supervisors execute an Agreement, with North Peninsula Neighborhood Services Center, in the amount of $102,097 for the term of July 1, 2005 to June 30, 2006, to provide CORE Safety Net Services; and

 

WHEREAS, on October 20, 2005, the Director of the Human Services Agency amended said Agreement to revise the payment structure to pay Contractor on a monthly basis; and

 

WHEREAS, there has been presented to this Board of Supervisors, for its consideration and acceptance, a Second Amendment to the Agreement with North Peninsula Neighborhood Services Center, for the purpose of partially restoring Safety Net Services providers to prior funding levels, this Amendment will increase the Agreement by $25,330 for a total obligation of $127,409, the term of the original Agreement shall remain July 1, 2005 to June 30, 2006; and

 

WHEREAS, this Board has been presented with a form of this Second Amendment and has examined and approved such Amendment as to both form and content and desires to enter into this Agreement.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the

1. President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment to the Agreement with Peninsula Neighborhood Services Center for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto; and

2. The Director of the Human Services Agency to execute subsequent amendments and minor modifications not to exceed an aggregate amount of $25,000, and make changes in the types of services and activities provided under the Agreement.
