RESOLUTION AUTHORIZING THE EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH LABORATORY CORPORATION OF AMERICA FOR THE TERM JANUARY 1, 2006 THROUGH JUNE 30, 2008 FOR A MAXIMUM OBLIGATION OF $675,000; AND AUTHORIZING THE DIRECTOR OF HEALTH TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000 IN AGGREGATE AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on December 28, 2005 the County Manager approved an agreement with Laboratory Corporation of America for the provision of clinical laboratory services for clients of Mental Health Services and Correctional Health Services for the term January 1, 2006 through June 30, 2006 for a maximum obligation of $100,000; and

WHEREAS, both parties now wish to amend the agreement to extend the term through June 30, 2008 and to increase the maximum obligation by $575,000 to a new maximum of $675,000; and

WHEREAS, this Board has been presented with the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement;

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment to said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Director of Health is authorized to execute subsequent amendments and minor modifications not to exceed $25,000 in aggregate and to make minor changes in the type of services and activities provided under the Agreement.
