RESOLUTION AUTHORIZING A FIRST AMENDMENT TO THE FLAT RATE ALCOHOL AND DRUG TREATMENT AND PREVENTION AGREEMENT WITH FREE AT LAST

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on August 29, 2005, by Resolution No. 067447, the County authorized execution of a flat rate Agreement (the “Original Agreement”) with Free at Last in the amount of $339,496, to provide Drug Treatment and Prevention Services, (the “Project”); and

WHEREAS, currently there is a need to increase the funding by $41,424 to provide for Men’s residential services; and

WHEREAS, both parties now wish to amend the Original Agreement to increase the funding by $41,424 for a new total obligation of $380,920; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a First Amendment to the Agreement with Free At Last to provide Flat Rate based alcohol and drug prevention and treatment services under contract with the County of San Mateo; and

WHEREAS, this Board has been presented with the form of this Amendment to the Agreement and has examined and approved it as to both form and content and desires to amend the Original Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the

President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
