RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT WITH

BAY AREA UROLOGY GROUP

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on June 29th, 2004 the Board of Supervisors, by Resolution 66774, authorized an Agreement with Bay Area Urology Group to provide urology services to the San Mateo Medical Center for the term June 1, 2004 through December 31, 2005; and

WHEREAS, the Board of Supervisors, by Resolution 67766, authorized an Amendment to the Agreement with Bay Area Urology Group, DPM , to extend the contract for urology services through February 28, 2006 while new contract terms were negotiated; and

WHEREAS, the San Mateo Medical Center has a continuous need for urology services and the Bay Area Urology Group has agreed to continue providing such services at the same level of compensation; and

WHEREAS, the parties now wish to enter into an Agreement for the term of March 1, 2006 through August 31, 2007 with an unchanged maximum reimbursement amount of $474,398.68; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made, whereby Bay Area Urology Group shall provide professional urology services; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made, whereby Bay Area Urology Group shall provide professional urology services; and

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
