RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH THE PENINSULA HEALTHCARE DISTRICT INCREASING THE AMOUNT BY $682,250 TO $2,114,500 AND EXTENDING THE TERM TO JANUARY 1, 2003 THROUGH DECEMBER 31, 2006

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, The Board of Supervisors, by resolution numbers 65792 and 67295, authorized an Agreement and Amendment with the Peninsula Healthcare District in the amount of $1,432,250 for the term January 1, 2003 through December 31, 2005 for funding for the Children’s Health Initiative;

WHEREAS, there has been presented to this Board of Supervisors for its consideration and approval, an Amendment to the Agreement, whereby the Peninsula Healthcare District will continue to support the Children’s Health Initiative;

WHEREAS, the parties wish to enter into an Amendment to the Agreement to provide funding for the Children’s Health Initiative increasing the amount by $682,250 to $2,114,500 and extending the term to January 1, 2003 through December 31, 2006:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said amendment for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
