RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH GIBSON & ASSOCIATES, INC. FOR THE PERIOD FEBRUARY 1, 2005 THROUGH JUNE 30, 2006, FOR A MAXIMUM OBLIGATION OF $109,170 AND AUTHORIZING THE DIRECTOR OF HEALTH TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000 AND TO MAKE MINOR CHANGES IN THE TYPE OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT |
RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that |
WHEREAS, on February 28, 2005, the County Manager approved an Agreement with Gibson & Associates for the period February 1, 2005 through June 30, 2006 for the facilitation of the planning process for services that will be funded by the California Mental Health Services Act (MHSA); and |
WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to that Agreement, reference to which is hereby made for further particulars, whereby Gibson & Associates shall provide services related to the implementation of the County Three-Year MHSA Plan; and |
WHEREAS, both parties now wish to amend the Agreement, to increase the maximum obligation by $17,105 for a total obligation of $109,170 and to expand the services provided; and |
WHEREAS, this Board has been presented with a form of the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the Amendment to the Agreement: |
NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is authorized to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto. |
BE IT FURTHER RESOLVED that the Director of Health is authorized to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000 and to make minor changes in the type of services and activities provided under the Agreement. |
