RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT WITH LAFRANCE ASSOCIATES TO PROVIDE PROGRAM EVALUATION SERVICES FOR THE FEDERALLY-FUNDED ADOLESCENT FAMILY LIFE PROJECT (AFLP) FOR THE TERM OF FEBRUARY 1, 2006 THROUGH DECEMBER 31, 2010, IN THE AMOUNT OF $165,000

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the Federal Office of Adolescent Pregnancy Programs has awarded a five year grant to San Mateo County AFLP with the requirement that an agreement with an independent program evaluator be developed; and

WHEREAS, LaFrance Associates was selected as a result of an RFP process to evaluate said program as required by grant guidelines; and

WHEREAS, both parties now wish to enter into an agreement to provide program evaluation services for the period of February 1, 2006 through December 31, 2010 , for a total obligation of $165,000; and

WHEREAS, this Board has been presented with a form of the agreement and has examined and approved it as to both form and content and desires to enter into said agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
