RESOLUTION AUTHORIZING THE WAIVING OF AN INVTIATION TO BID (ITB) PROCESS, AUTHORIZING THE PURCHASING AGENT TO AMEND VENDOR AGREEMENT # B606025 FOR LABORATORY SUPPLIES WITH ROCHE DIAGNOSTIC CORPORATION AND AUTHORIZING THE PURCHASING AGENT TO ISSUE A PURCHASE ORDER FOR LABORATORY EQUIPMENT WITH ROCHE DIAGNOSTIC CORPORATION FOR THE TERM OF JULY 1, 2005 THROUGH DECEMBER 31, 2007, IN THE AMOUNT OF $344,473

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a request to allow the Purchasing Agent to amend a Vendor Agreement and issue a purchase order with Roche Diagnostic Corporation, reference to which is hereby made for further particulars; and

WHEREAS, both parties now wish to amend the Vendor Agreement to provide supplies through December 31, 2007 and to increase the amount by $109,650, for a total obligation of $344,473, to allow for the purchase of a LightCycler system; and

WHEREAS, this Board has been presented with this request to allow the Purchasing Agent to execute said Amendment and issue an purchase order for laboratory supplies and equipment and has approved it as to both form and content and desires to enter into said Amendment;

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board of Supervisors be, and is hereby authorized to waive the Invitation to Bid (ITB) process; authorize the Purchasing Agent to execute an Amendment to the vendor agreement for laboratory supplies with Roche Diagnostic Corporation; and authorize the Purchasing Agent to issue a purchase order for laboratory equipment with Roche Diagnostic Corporation for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
