RESOLUTION AUTHORIZING 1) THE EXECUTION OF AGREEMENT NO. AP-0607-08 FOR TITLE III/VII OF THE OLDER AMERICANS ACT (OAA) AND FOR THE COMMUNITY-BASED SERVICES PROGRAM (CBSP) OF THE OLDER CALIFORNIANS ACT (OCA) WITH THE CALIFORNIA DEPARTMENT OF AGING (CDA) FOR FISCAL YEAR 2006-07 IN THE AMOUNT OF $3,058,305; AND 2) THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, this Board has designated itself as the Area Agency on Aging of San Mateo County to carry out programs supporting senior services;

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby the Aging and Adult Services Division shall provide Adult Day Care, Alzheimer’s Program, Brown Bag, Case Management, Congregate Nutrition, Disease Prevention/Health Promotion, Family Caregiver Support, Home-Delivered Meals, Legal Assistance, Ombudsman Program, Transportation Program, and Unsubsidized Senior Employment services and;

 

WHEREAS, both parties now wish to enter into an Agreement to provide Title III/VII and Community-Based Serves Programs (CBSP) funding for Older Americans Act and Older Californians Act programs for the period of July 1, 2006 through June 30, 2007, for a total obligation of $3,058,305:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board of Supervisors be, and is hereby authorized and directed to execute said Agreement with the California Department of Aging for the Title III/VII and CBSP for FY 2006-2007 for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Director of the Health Department is hereby authorized to execute subsequent Amendments and minor modifications to this Agreement not to exceed the aggregate of $25,000 for the term of the Agreement provided the additional amount is within budgetary appropriations and the term is not extended.
