RESOLUTION AUTHORIZING 1) THE EXECUTION OF AN AGREEMENT WITH THE CALIFORNIA DEPARTMENT OF AGING, NUMBER MS-0607-13 IN THE AMOUNT OF $739,385 FOR THE CONTINUATION OF MULTIPURPOSE SENIOR SERVICES PROGRAM (MSSP) SERVICES FOR FISCAL YEAR 2006-07, AND 2) THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby the Aging and Adult Services Division shall provide a Multipurpose Senior Services Program (MSSP) that provides comprehensive social and health support necessary to enable frail elderly to remain in their own homes;

WHEREAS, the parties wish to enter into an Agreement to provide MSSP services for the period of July 1, 2006 through June 30, 2007, for a total obligation of $739,385; and

WHEREAS, this Board has examined the Agreement and has approved it as to both form and content:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board of Supervisors be and is hereby authorized and directed to execute said Agreement Number MS-0607-13 for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

IT IS HEREBY FURTHER RESOLVED that the Director of the Health Department shall be authorized to execute subsequent amendments and minor modifications not to exceed $25,000 during the term of the Agreement.
