RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO

A) EXECUTE AN AGREEMENT WITH MENTAL HEALTH ASSOCIATION OF SAN MATEO COUNTY IN THE AMOUNT OF $712,724 TO PROVIDE HIV HOUSING SERVICES AND MANAGE THE WRAP-AROUND FUND FOR THE PERIOD OF JULY 1, 2006 TO JUNE 30, 2007, AND B) AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby Mental Health Association of San Mateo County shall manage the wrap-around fund and provide HIV housing services to clients of the San Mateo County AIDS Program; and

WHEREAS, the term of said Agreement is July 1, 2006 through June 30, 2007, and the maximum dollar amount is $712,724; and

WHEREAS, this Board has been presented with the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED, that the Director of the Health Department is hereby authorized to execute minor amendments and modifications to the Agreement not to exceed an aggregate of $25,000, and to make minor changes in the types of services and activities provided under the Agreement.
