RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD OF SUPERVISORS TO EXECUTE AN AMENDMENT TO THE GRANT AGREEMENT WITH THE CALIFORNIA DEPARTMENT OF HEALTH SERVICES (DHS) FOR BIOTERRORISM PREPAREDNESS AND RELATED ACTIVITES IN THE AMOUNT OF $82,848 FOR THE PERIOD OF MAY 22, 2006 THROUGH JUNE 30, 2006

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, your Board approved the California Department of Health Services grant agreement with San Mateo County for 2005-06 for the period of August 31, 2005 to August 30, 2006, for a total award amount of $848,209 on May 2, 2006; and

WHEREAS, California Department of Health Services has presented an amendment to the grant agreement to San Mateo County for 2005-06 for the period of May 22, 2006 to June 30, 2006, in the amount of $82,848 for pandemic influenza preparedness activities; and

WHEREAS, both parties now wish to enter into said amended agreement :

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said amendment to the grant agreement with the California Department of Health Services for pandemic influenza preparedness activities for and on behalf of the County of San Mateo and the Clerk of this Board shall attest the President's signature thereto.
