RESOLUTION A) AUTHORIZING THE PRESIDENT OF THE BOARD TO

EXECUTE AGREEMENTS WITH EL CONCILIO OF SAN MATEO COUNTY FOR $112,991 AND WITH FREE AT LAST FOR $178,139 FOR THE PERIOD OF JULY 1, 2006 TO JUNE 30, 2007 TO PROVIDE PREVENTION CASE MANAGEMENT AND HIV STREET OUTREACH AND TESTING, AND B) AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000 FOR EACH AGREEMENT AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENTS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance Agreements, reference to which is hereby made for further particulars, whereby El Concilio of San Mateo County and Free at Last shall provide prevention case management and HIV street outreach and testing services; and

WHEREAS, the term of said Agreements is July 1, 2006 through June 30, 2007, and the maximum dollar amounts are $112,991 for El Concilio of San Mateo County and $178,139 for Free at Last; and

WHEREAS, this Board has been presented with the Agreements and has examined and approved them as to both form and content and desires to enter into the Agreements.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute the aforesaid Agreements for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED, that the Director of the Health Department is hereby authorized to execute minor amendments and modifications to the Agreements not to exceed $25,000 for each Agreement, and to make minor changes in the types of services and activities provided under the Agreements.
