RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH CHILDREN’S HEALTH COUNCIL, INC. FOR THE PERIOD JUNE 1, 2006 THROUGH JUNE 30, 2006, TO INCREASE THE AGREEMENT MAXIMUM OBLIGATION BY $45,000 TO A NEW MAXIMUM OF $504,064

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on December 14, 2004, this Board of Supervisors approved an Agreement, whereby Children’s Health Council, Inc. shall provide non-public school-based mental health services for mentally ill children and youth for the period July 1, 2004 through June 30, 2005, for a maximum obligation of $205,313; and

WHEREAS, on December 13, 2005, this Board of Supervisors approved an Amendment to this Agreement, extending the term through June 30, 2006, and increasing the maximum obligation to $459,064; and

WHEREAS, both parties now wish to further amend the Agreement, to add the provision of Mental Health Services authorized by the County ACCESS Team for the period June 1, 2006 through June 30, 2006, and increasing the maximum obligation to $504,064; and

WHEREAS, this Board has been presented with a form of the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the Amendment to the Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is authorized to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
