BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING: 1) THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH TELECARE CORPORATION FOR FULL SERVICE PARTNERSHIP MENTAL HEALTH SERVICE PROGRAMS FOR THE HIGHEST RISK ADULTS AND HIGHEST RISK OLDER ADULTS TO EXTEND THE TERM THROUGH JUNE 30, 2008, AND TO INCREASE THE AGREEMENT MAXIMUM BY $4,138,459 TO A NEW MAXIMUM OF $4,238,459, AND 2) THE DIRECTOR OF HEALTH TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on May 10, 2006 the County Manager approved an agreement with Telecare Corporation to perform start-up services for the implementation of full service partnership mental health service programs for the highest risk adults and highest risk older adults / medically fragile adults for the term April 16, 2006 through June 30, 2006; and

 

WHEREAS, both parties now wish to amend the Agreement for the provision of full service partnership services to increase the amount by $4,138,459 for a new total obligation of $4,238,459 and extend the term through June 30, 2008; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an amendment to the Agreement, reference to which is hereby made for further particulars, whereby Telecare Corporation shall provide full service partnership services for adults and older adults/medically fragile adults; and

WHEREAS, this Board has been presented with a form of the amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the amendment to the Agreement:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is authorized to execute said amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Director of Health is authorized to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000 and to make minor changes in the type of services and activities provided under the Agreement.
