BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING (A) WAIVER OF THE REQUEST FOR PROPOSAL (RFP) PROCESS; (B) EXECUTION OF AN AGREEMENT WITH YOUTH LEADERSHIP INSTITUTE (YLI) FOR TOBACCO PREVENTION SERVICES; AND (C) THE DIRECTOR OF THE HUMAN SERVICES AGENCY TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000, AND TO MAKE CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THIS AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, there has been presented to this Board of Supervisors a request

to wave the Request for Proposal (RFP) process; and it is the best interest of the County to grant such waiver because the County previously contracted with Youth Leadership Institute following an RFP for tobacco control services and YLI successfully performed phase one of the process; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance of an Agreement with Youth Leadership Institute to provided phase two tobacco prevention services for the term of July 1, 2006 through June 30, 2007 with a maximum obligation of $145,000; and

 

WHEREAS, this Board has been presented with a form of this Agreement and has examined and approved such Agreement as to both form and content and desires to enter into this Agreement.

 

    NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that

    A. The Request for Proposal process is waived; and

    B. The President of the Board of Supervisors be, and is hereby, authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest to the President’s signature thereto; and

    C. The Director of the Human Services Agency shall be authorized to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000, and to make minor changes in the types of services and activities provided under the Agreement.
