RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT
WITH THE REGENTS OF THE UNIVERSITY OF CALIFORNIA TO PROVIDE PLASTIC SURGERY SERVICES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on April 12, 2005 the Board of Supervisors, by Resolution 67255, authorized an Agreement in the amount of $161,158.33 for the thirteen month term of June 1, 2004 through June 30, 2005 for the purpose of providing plastic surgery services to the San Mateo Medical Center; and

WHEREAS San Mateo Medical Center’s has a continued need for post operative and reconstructive plastic surgery services, and

WHEREAS, agreement on a new contract for plastic surgery services in FY0506 have been delayed due to prolonged negotiations over reimbursement terms and services; and

WHEREAS, both parties now wish to enter into an Agreement to provide plastic surgery services for the period of July 1, 2005 through June 30, 2006 for a total obligation of $157,508.10; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby The Regents of the University of California, shall provide plastic surgery services; and

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
