RESOLUTION AUTHORIZING THE EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH GE MEDICAL SYSTEMS FOR PREVENTATIVE MAINTENANCE AND SERVICE OF GENERAL BIOMEDICAL AND IMAGING EQUIPMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on December 18, 2001, your Board, by Resolution Number 64962, authorized a consolidated three-year Agreement with GE Medical Systems for preventative maintenance and repair of imaging and biomedical equipment, in the amount of $1,701,336, for the term January 1, 2002 to December 30, 2004; and

WHEREAS, on January 4, 2005, your Board, by Resolution Number 67136, authorized a first Amendment to increase the Agreement amount by $680,000, from $1,701,336 to $2,381,336, and to extend the term by one year to give San Mateo Medical Center the opportunity to form a multi disciplinary committee to develop a RFP for this service. The increase included the costs of coverage for the new equipment at SMMC, BLTC, and the Women and Children Services, as well as the cost of coverage of the one-year extension; and

WHEREAS, the parties wish to further amend the Agreement by increasing the amount by $59,317, changing the Agreement obligation from $2,381,336 to $2,440,653 for the purpose of paying final invoices GE Medical Systems; and

WHEREAS, the Board has been presented with this Amendment and desires to enter into same.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED, that the President of the Board of Supervisors is hereby authorized and directed to execute said Amendment to the Agreement for and on Behalf of the County of San Mateo and the Clerk of the Board shall attest the President’s signature thereto.
