RESOLUTION AUTHORIZING: 1) THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH THE MENTAL HEALTH ASSOCIATION OF ALAMEDA COUNTY FOR THE PROVISION OF PATIENTS’ RIGHTS SERVICES FOR THE TERM JULY 1, 2006 THROUGH JUNE 30, 2009 FOR A MAXIMUM AMOUNT OF $581,855; AND 2) AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000 AND TO MAKE MINOR CHANGES IN THE TYPE OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made by further particulars, whereby the Mental Health Association of Alameda County shall provide advocacy services on behalf of the Mental Health Services Division of the Health Department;

WHEREAS, both parties now wish to enter into an Agreement to provide patient advocacy services for the period of July 1, 2006 through June 30, 2009, for a total obligation of $581,855; and

WHEREAS, this Board has been presented with the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED, the Director of the Health Department shall be authorized to execute subsequent amendments and minor modifications to the Agreement not to exceed an aggregate of $25,000 and to make minor changes in the types of services and activities under the Agreement.
