RESOLUTION 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH THE CENTER TO PROMOTE HEALTHCARE ACCESS TO PROVIDE ONE-E-APP MAINTENANCE AND SYSTEM ENHANCEMENT SERVICES FOR THE TERM OF JULY 1, 2006 THROUGH JUNE 30, 2007 FOR A TOTAL OBLIGATION NOT TO EXCEED $299,779; 2) AUTHORIZING THE DIRECTOR OF HEALTH TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby The Center to Promote HealthCare Access will provide installation, configuration, implementation, maintenance, enhancement and support services of the One-e-App system; and,

WHEREAS, the term of the Agreement is July 1, 2006 through June 30, 2007, for a total obligation of $299,779; and,

WHEREAS, this Board of Supervisors has been presented with a form of the Agreement and has examined and approved the Agreement as to form and content and desires to enter into the Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo and the Clerk of this Board shall attest the President’s signature thereto.

BE IT RESOLVED that the Director of Health is authorized to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000 and to make minor changes in the type of services and activities provided under the Agreement.
