RESOLUTION (1) AUTHORIZING AN AMENDMENT TO THE AGREEMENT WITH THE STATE DEPARTMENT OF HEALTH SERVICES FOR A LEAD HAZARD REDUCTION COMPLIANCE AND ENFORCEMENT PROGRAM EXTENDING THE CONTRACT TERMINATION DATE TO JUNE 30, 2007; AND (2) AUTHORIZING THE HEALTH DEPARTMENT DIRECTOR TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT RELATING TO THE LEAD HAZARD REDUCTION COMPLIANCE AND ENFORCEMENT PROGRAM.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on June 8, 2004, the Board of Supervisors, by Resolution No. 66693, authorized an Agreement in the amount of $108,072 for the term of February 1, 2004 through June 30, 2006 for the purpose of the development and implementation of a County-wide infrastructure and framework for a successful Lead Hazard Reduction Compliance and Enforcement Program to help protect the health of County residents; and

WHEREAS, the term of the Agreement has expired and the parties wish to extend the term of the Agreement from July 1, 2006 through June 30, 2007 with no change in the original contract amount of $108,072; and

WHEREAS, this Board has been presented with the Amendment and has examined and approved it as to both form and content and desires to enter into the Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors is hereby authorized and directed to execute said amendment to the Agreement and authorizes the Health Department Director to execute all subsequent amendments and minor modifications not to exceed an aggregate of $25,000 and to make minor changes in the types of services and activities provided under the Agreement related thereto for and on behalf of the County of San Mateo and the Clerk of the Board shall attest the President’s signature thereto.
