RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH CAMINAR FOR THE PROVISION OF HOUSING AND HOUSING RELATED SERVICES, SUPPORTED EDUCATION SERVICES, INTENSIVE CASE MANAGEMENT AND REHABILITATION SERVICES FOR THE PERIOD JULY 1, 2005 THROUGH JUNE 30, 2006, TO INCREASE THE AGREEMENT MAXIMUM OBLIGATION BY $38,695 TO A NEW MAXIMUM OBLIGATION OF $5,740,374

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on October 4, 2005, this Board of Supervisors approved an Agreement, whereby Caminar shall provide housing and housing related services, supported education services, intensive case management and rehabilitation services for mentally ill clients for the period July 1, 2005 through June 30, 2006, for a maximum obligation of $5,701,679; and

WHEREAS, both parties now wish to amend the Agreement to add the provision of enhanced supported education services for transition-age youth and to increase the maximum obligation by $38,695 to a new maximum obligation of $5,740,374; and

WHEREAS, this Board has been presented with a form of the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the Amendment to the Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
