RESOLUTION (A) AUTHORIZING A SECOND AMENDMENT TO THE AGREEMENT WITH KATE BRISTOL CONSULTING TO PROVIDE CONSULTING SERVICES AND TECHNICAL SUPPORT FOR THE SAN MATEO COUNTY HOMELESS CONTINUUM OF CARE (COC), FOR THE TERM OF AUGUST 1, 2004 TO JULY 31, 2007, INCREASING THE AMOUNT BY $50,000 TO $125,000, $50,000 IS NET COUNTY COST; AND (B) AUTHORIZING THE DIRECTOR OF THE HUMAN SERVICES AGENCY TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

 

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on September 17, 2004, the County entered into an Agreement with Kate Bristol Consulting in the amount of $50,000 for the term of August 1, 2004 to July 31, 2007 to provide technical assistance to the Human Services Agency necessary for the San Mateo County Homeless Continuum of Care planning process and plan development; and

 

WHEREAS, on September 22, 2005, the County entered into Change Order No. 1 to the Agreement with Kate Bristol Consulting to increase the amount by $25,000, for a total obligation of $75,000, to continue to provide to the Human Services Agency necessary technical assistance for the San Mateo County Homeless Continuum of Care application development and submission process, as well as to utilize the Contractor’s services in connection with development of funding applications and other projects related to homeless services; and

 

WHEREAS, the County now wishes to enter into a Second Amendment to the Agreement with Kate Bristol Consulting to increase the amount by $50,000, for a total obligation of $125,000, for the Contractor to continue to provide consultation and technical support for the Continuum of Care’s year-round planning and developing related measures and, in addition, to provide for more detailed technical support; and

 

WHEREAS, the term of this Agreement remains August 1, 2004 through July 31, 2007; and

 

WHEREAS, this Board has been presented with a form of this Second Amendment and has examined and approved it as to both form and content and desires to enter into the same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that:

A. The President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto; and

 

B. The Director of the Human Services Agency is hereby authorized to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000 and to make minor changes in the types of services and activities provided under the Agreement.
