RESOLUTION AUTHORIZING AN AGREEMENT WITH THE SAN MATEO COMMUNITY HEALTH AUTHORITY FOR HEALTH BENEFITS TO EXTRA HELP EMPLOYEES FOR THE PERIOD FROM SEPTEMBER 1, 2006 THROUGH AUGUST 31, 2011

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the County has agreed in negotiations with extra help personnel to provide managed health care services; and

WHEREAS, the San Mateo Community Health Authority has agreed to provide its HealthWorx Plan to extra help County employees; and

WHEREAS, the County has been presented with a form of such agreement for provision of managed health care benefits to eligible individuals:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby authorized, to sign for an on behalf of the County of San Mateo and the Clerk of the Board shall attest the President’s signature thereto.
