BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING: A) THE WAIVER OF THE REQUEST FOR PROPOSAL PROCESS; B) A FORTY-TWO (42) MONTH AGREEMENT WITH THE REGENTS OF THE UNIVERSITY OF CALIFORNIA SAN FRANCISCO (UCSF) TO PROVIDE ALCOHOL AND DRUG TREATMENT EVALUATION SERVICES FOR THE TERM OF AUGUST 15, 2005 TO FEBRUARY 14, 2009, IN THE AMOUNT OF $145,500; AND C) THE DIRECTOR OF THE HUMAN SERVICES AGENCY TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000, AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

 

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, there has been presented to this Board of Supervisors, for its consideration and acceptance, an Agreement with the Regents of the University of California San Francisco (UCSF) to provide alcohol and drug treatment evaluation services; and

 

WHEREAS, the term of this Agreement is August 15, 2005 through February 14, 2009, and the amount of the Agreement is $145,500; and

 

WHEREAS, the Board concludes that it is in the best interests of the County to waive the Request For Proposals (RFP) process with respect to the services to be provided because UCSF and HSA jointly developed and submitted the grant proposal providing funding for the treatment services to be evaluated and because UCSF was identified in the funded proposal as the evaluator; and

 

WHEREAS, this Board has been presented with the form of the Agreement and has examined and approved the Agreement as to both form and content and desires to enter into the Agreement.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Request for Proposal Process is waived; and

 

BE IT FURTHER RESOLVED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto; and

 

BE IT FURTHER RESOLVED that the Director of the Human Services Agency shall be authorized to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000, and to make minor changes in the types of services and activities provided under the Agreement.
