BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE EXECUTION OF A SECOND AMENDMENT WITH DOCUMENT FULFILLMENT SERVICES TO PROVIDE THE PROCESSING OF CALWIN DOCUMENTATION, AND EXTENDING THE TERM TO DECEMBER 31, 2006, INCREASING THE AMOUNT BY $120,000, FOR A TOTAL OBLIGATION OF $220,000

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on September 17, 2005, the County entered into an Agreement with Document Fulfillment Services in the amount of $81,037, for the term of October 1, 2005 to September 30, 2006, to provide the following services: 1) Daily pickup of data DVD disks from the EDS location in Rancho Cordova, 2) Printing of all documents, 3) Folding, 4) Sorting, and 5) Mailing via US Postal Service; and

WHEREAS, on June 12, 2006, the parties entered into Change Order No. 1 to the Agreement with Document Fulfillment Services to increase the amount by $18,963, for a total obligation of $100,000, to providing additional contract funding due to the unanticipated large volume of printing and additional US Postage costs that had been necessary to process client correspondence; and

WHEREAS, the parties now wish to further amend the Agreement with Document Fulfillment Services to increase the amount by $120,000, for a total contract obligation of $220,000, as a result of continuing unanticipated large volume of printing that has been necessary to process client correspondence and increased US Postage costs and to extend the term to December 31, 2006; and

WHEREAS, the new term of this Agreement will be October 1, 2005 to December 31, 2006; and

WHEREAS, this Board has been presented with a form of this Second Amendment and has examined and approved it as to both form and content and desires to enter into the same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
