RESOLUTION 1) AUTHORIZING THE EXECUTION OF AN AGREEMENT WITH INFOIMAGE, INC., TO PROCESS, PRINT AND MAIL STATEMENTS FOR ENVIRONMENTAL HEALTH, MENTAL HEALTH AND SAN MATEO MEDICAL CENTER FOR THE TERM JULY 1, 2006 TO JUNE 30, 2008, IN AN AMOUNT NOT TO EXCEED $395,823, AND 2) AUTHORIZING THE CHIEF EXECUTIVE OFFICER OF SAN MATEO MEDICAL CENTER TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000 AND TO MAKE MINOR CHANGES IN THE TYPE OF SERVICES AND ACTIVITIES
PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, In May 2006, a Request for Proposal Process (RFP) was conducted for the service to process, print and mail statements for Environmental Health, Mental Health and San Mateo Medical Center; and

 

WHEREAS, the RFP was sent to four vendors, two of whom responded. InfoImage was selected based on cost savings, responsiveness, and sufficient resources to perform the required services to Environmental Health, Mental Health, and San Mateo Medical Center; and

 

WHEREAS, the Agreement with InfoImage is for two years with a 60 day termination clause; and

 

WHEREAS, the parties wish to enter into a new Agreement with InfoImage to provide laser printing, mailing, and programming services to the Health Services Department and to San Mateo Medical Center, for the period July 1, 2006 to June 30, 2008, for a total obligation of $395,823; and

 

WHEREAS, the Board has been presented with an Agreement between the County of San Mateo and InfoImage, Inc. and desires to enter into the same.

 

NOW THEREFORE, IT IS HEREBY RESOLVED that the Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Chief Executive Officer of San Mateo Medical Center shall be authorized to execute subsequent Amendments and minor modifications not to exceed $25,000 and to make minor changes in the type of services and activities provided under the Agreement.
