RESOLUTION WAIVING THE RFP PROCESS AND AUTHORIZING AN EXTENSION OF AN AGREEMENT WITH AIG FOR ACCIDENTAL DEATH AND DISMEMBERMENT BENEFITS FOR THE PERIOD FROM OCTOBER 1, 2006 THROUGH OCTOBER 1, 2009 FOR AN ESTIMATED ANNUAL COST OF $28,000

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the County has established an agreement with AIG to provide Accidental Death and Dismemberment benefits; and

WHEREAS, the County has proposed a continuation of services a no premium increase to the County or employees; and

WHEREAS, the County wishes to accept the proposed continuation of services without a Request for Proposal process and has been presented with a form of such agreement for continuation of Accidental Death and Dismemberment benefits for eligible employees:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board of Supervisors waives the RFP process and that the President be and is hereby authorized and directed to sign for an on behalf of the County of San Mateo and the Clerk of the Board shall attest the President’s signature thereto.
