RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD OF SUPERVISORS TO (1) EXECUTE AN AGREEMENT WITH CH MACK, INC., FOR THE PROVISION OF AN AUTOMATED CASE MANAGEMENT SYSTEM AND ASSESSMENT TOOL, FOR THE TERM OF OCTOBER 1, 2006 THROUGH SEPTEMBER 30, 2008 WITH THE OPTION TO RENEW FOR ONE, ONE-YEAR TERM, FOR A TOTAL OBLIGATION OF $471,072, AND (2) AUTHORIZE THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an agreement, reference to which is hereby made for further particulars, whereby CH Mack, Inc., will provide an automated case management system for the programs in Aging and Adult Services, and

WHEREAS, both parties now wish to enter into said Agreement to provide an automated case management system and assessment tool, for the term October 1, 2006 through September 30, 2008 with the option to renew for one, one-year term, for a total obligation of $471,072:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board be, and is hereby authorized and directed to execute said Agreement with CH Mack, Inc., for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Director of the Health Department is hereby authorized to execute subsequent Amendments and minor modifications to this Agreement not to exceed the aggregate of $25,000 and to make minor changes in the types of services and activities provided under the agreement.
