RESOLUTION 1) AUTHORIZING AN AMENDMENT TO THE AGREEMENT WITH THE SAN MATEO HEALTH COMMISSION AT A NEW RATE NOT TO EXCEED $91.45 PER-MEMBER PER-MONTH EFFECTIVE FEBRUARY 12, 2006, ADDING $56,653 FOR RETENTION EFFORTS, AND CHANGING THE CONTRACTING AGENCY NAME FROM THE SAN MATEO HEALTH COMMISSION TO THE SAN MATEO COMMUNITY HEALTH AUTHORITY; 2) AUTHORIZING THE DIRECTOR OF HEALTH TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on January 28, 2003 the Board of Supervisors, by Resolution Number 065792, approved an Agreement for the period January 28, 2003 to February 11, 2005 whereby the San Mateo Health Commission will support the Children’s Health Initiative by administering health insurance medical, dental, and vision benefits for Healthy Kids eligible children; and

 

WHEREAS, on May 24, 2005, the Board of Supervisors, by Resolution Number 067335, approved an Agreement for the period February 12, 2005 to February 11, 2008, to provide health insurance to Healthy Kids members; and

 

WHEREAS, both parties now wish to amend the Agreement to provide health insurance to Healthy Kids members at a new rate not to exceed $91.45 per-member per- month, adding $56,653 for retention efforts, and changing the name of the contracting agency from the San Mateo Health Commission to the San Mateo Community Health Authority; and

 

WHEREAS, this Board has been presented with this Amendment to the Agreement and has approved it as to both form and content, and desires to enter into said Agreement:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute the said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto; and

 

BE IT RESOLVED that the Director of Health is authorized to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000 and to make minor changes in the types of services and activities provided under the Agreement.
