RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT WITH LARRY K. FAN, M.D. INC. TO PROVIDE PLASTIC SURGERY SERVICES FOR THE PERIOD SEPTEMBER 1, 2006 THROUGH AUGUST 31, 2007 IN THE AMOUNT OF $124,500

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on June 20, 2006 the Board of Supervisors, by Resolution 68093, authorized an Agreement with the Regents of the University of California in the amount of $157,508.10 for the term of July 1, 2005 through June 30, 2006 for the purpose of providing plastic surgery services to the San Mateo Medical Center; and

WHEREAS the Regents of the University of California have decided not to renew their contract and the San Mateo Medical Center has a continued need for post operative and reconstructive plastic surgery services, and

WHEREAS, Larry K. Fan, M.D. Inc. has been selected as one of two providers to replace the Regents of the University of California under a new contract for post operative and reconstructive plastic surgery services; and

WHEREAS, both parties now wish to enter into an Agreement to provide plastic surgery services for the period of September 1, 2006 through August 31, 2007 for a total obligation of $124,500; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby Larry K. Fan, M.D. Inc. shall provide plastic surgery services; and

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
