RECOMMENDATION:
|
Adopt a Resolution authorizing the execution of an Amendment to the Agreement with Onsite Dental Care Foundation to provide dental services to San Mateo County’s homeless population, amending in their entirety Exhibits A and B, increasing the maximum amount payable by $113,035 to $311,610, extending the term of the Agreement by six months to April 30, 2007, and adding the Ordinance Code’s Jury Duty provisions to the Agreement.

BACKGROUND:
|
San Mateo Medical Center (SMMC) is annually awarded the Health Care for the Homeless (HCH) grant from the federal Bureau of Primary Health Care of the Health and Resource Services Administration to provide primary care, dental care, outreach, health education, case management, and supportive services to the San Mateo County’s homeless population.
HCH is designed to assist homeless adults, families and youth with access to and use of the SMMC’s continuum of primary care, dental care, mental health, substance abuse, and other support services. In Winter 2003, the HCH Advisory Committee developed a strategic plan for the HCH Program. The goals of the Strategic Plan include to: 1) Serve a large portion of the homeless population who are on the street or in shelters; 2) Provide one or more of the following services to homeless individuals: primary care services, dental care services, substance abuse and mental health treatment; 3) Eliminate barriers and provide better access to services by outreach to homeless individuals.
Following a Request for Proposal process, Onsite Dental Care Foundation was one of six contractors awarded an agreement based on the priorities of the strategic plan. The term of the Agreement with Onsite Dental Care Foundation was November 1, 2005 to October 31, 2006, with a maximum amount payable under the Agreement of $198,575.

DISCUSSION:
|
The Health Care for the Homeless (HCH) Program is extending this contract for an additional six months because it is in the process of designing a Request for Proposal (RFP) that will be issued in January 2007. The HCH Program has created a Business Plan which has allowed it to analyze how it currently provides services to the homeless population in San Mateo County. As the result of the Business Plan, the Program is working on restructuring its service delivery system so this population receives high quality services with greater access to them. Once the service system is restructured a RFP will be issued so organizations that serve individuals who are homeless can submit proposals and apply for funding to provide medical, dental, mental health, substance abuse and/or other case management services. This RFP Process will be completed by April 2007 and contracts will be awarded for the period beginning May 1, 2007.
The contractor has assured compliance with the County's Contractor Employee Jury Service Ordinance, as well as all other contract provisions that are required by County ordinance and administrative memoranda, including but not limited to insurance, hold harmless, non-discrimination and equal benefits.
County Counsel and Risk Management have reviewed and approved this Agreement.

FISCAL IMPACT:
|
This Amendment extends term of this Agreement by six months, through April 30, 2007. The maximum amount of this Agreement is increased by $113,035, from $198,575 to $311,610. The volume of patients seen has increased and is not mathematically proportional to patients seen in the previous 12 month period. All funds for this Agreement are provided by the HCH grant. Expenditures and grant revenue for this Agreement are included in San Mateo Medical Center’s FY2006-07 Adopted Budget. There is no Net County Cost.
