AMENDMENT TO AGREEMENT
BETWEEN THE COUNTY OF SAN MATEO AND
SEQUOIA ANESTHESIA CONSULTANTS

THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and Sequoia Anesthesia Consultants, hereinafter called "Contractor";
|
|
|
W I T N E S S E T H:
|
|
|
WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;
|
|
|
WHEREAS, pursuant to Resolution No. 67699 the parties entered into an Agreement, dated December 6, 2005 for anesthesia services at San Mateo Medical Center for the period of November 1, 2005 through October 31, 2006; and
|
|
|
WHEREAS, the parties wish to amend the Agreement to extend the contract period and the maximum amount payable thereunder;
|
|
|
NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:
|

Paragraph 3.1 of the Agreement is hereby amended in its entirety to read as follows:
This Agreement shall commence on the 1st day of November, 2005 and, unless sooner terminated, shall continue in force through April 30, 2007.
Paragraph 4 of Schedule B (Payments) of the Agreement is hereby amended in its entirety to read as follows:
Contractor’s quarterly compensation will be no less than a minimum amount listed below:
Monthly = $117,500
November 1, 2005 through January 31, 2006: $352,500
February 1, 2006 through April 30, 2006: $352,500
May 1, 2006 through July 31, 2006: $352,500
August 1, 2006 through October 31, 2006: $352,500
November 1, 2006 through January 31, 2007: $352,500
February 1, 2006 through April 30, 2007: $352,500

The last paragraph of Schedule B (Payments) of the Agreement is hereby amended in its entirety to read as follows:
Total payments for services under this Agreement will not exceed TWO MILLION FOUR HUNDRED AND NINETY THOUSAND DOLLARS ($2,490,000.00).

|
4.

All other terms and conditions of the Agreement shall remain in full force and effect.
|
|
|
