RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH SEQUOIA ANESTHESIA CONSULTANTS THAT EXTENDS THE CURRENT AGREEMENT FOR ANESTHESIOLOGY AND PAIN MANAGEMENT SERVICES FROM NOVEMBER 1, 2006 THROUGH APRIL 30, 2007 AND WHICH INCREASES THE MAXIMUM AMOUNT PAYABLE UNDER THE AGREEMENT BY $830,000 FROM $1,660,000 TO $2,490,OOO

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on December 6, 2005, by Resolution No. 67699, this Board authorized the County to enter into an Agreement with Sequoia Anesthesia Consultants for anesthesia and pain management services for the term of November 1, 2005 to October 31, 2006, with a maximum reimbursement amount of $1,660,000; and

WHEREAS, the parties now wish to amend the Agreement to extend its term by six months, until April 30, 2007 and to increase the maximum reimbursement amount there under by $830,000, to $2,490,000; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the Agreement, reference to which is hereby made for further particulars , whereby Sequoia Anesthesia Consultants shall provide professional anesthesiology and pain management services; and

WHEREAS, this Board has examined and approved the form of Amendment as to both form and content and desires to enter it.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
