ADOPT A RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH THE URBAN INSTITUTE TO EVALUATE THE CHILDREN’S HEALTH INITIATIVE, INCREASING THE AMOUNT TO $1,378,538 FOR THE TERM APRIL 29, 2003 THROUGH APRIL 28, 2008

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on September 13, 2005, the Board of Supervisors by Resolution Number 067535 authorized an Amendment to Agreement Number 066093 in the amount of $1,338,931 for the term April 29, 2003 through April 28, 2008, with The Urban Institute to provide evaluation services for the Children’s Health Initiative; and

WHEREAS, the parties wish to amend the Agreement by increasing the amount by $39,607 to $1,378,538, with no change to the term; and

WHEREAS, this Board has been presented with this Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into said Amendment:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
