BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING AN AMENDMENT TO THE AGREEMENT WITH FREE AT LAST TO PROVIDE ADDITIONAL FUNDING FOR MEN’S RESIDENTIAL SERVICES, THE TERM REMAINS THE SAME, JULY 1, 2006 THROUGH JUNE 30, 2007, INCREASING THE AMOUNT BY $82,848 TO $515,377

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on August 1, 2006, by Resolution No. 068187, the Board of Supervisors authorized execution of an Agreement with Free at Last in the amount of $432,529, to provide Drug Treatment and Prevention Services; and

WHEREAS, the parties now wish to amend the Agreement to provide for men’s residential services and increase the funding by $82,848 for a new total obligation of $515,377; and

WHEREAS, the Board has been presented with forms of this Amendment to the Agreement and the Board has examined and approved same as to both form and content and desires to enter into said Amendment; and

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
